a formulary that is determined and reviewed by an independent Pharmacy & Therapeutics
(P&T)
committee. The P&T Committee evaluates clinical information based on
medical literatures and current national treatment guidelines. Only after
the product is deemed safe and effective will the product be entertained
for formulary status.
